Sinopsis
Semalam Madoka bermimpi aneh, ia melihat seorang gadis layaknya penyihir sedang bertarung mati-matian dan sesosok familiar aneh memberitahukannya bahwa Madoka memiliki kekuatan untuk membantunya. Dan benar, keesokan harinya gadis yang terlihat dimimpinya muncul di sekolahnya.
Gadis tersebut ialah Homura Akemi, ia terlihat seperti gadis normal, namun setelah pulang dari sekolah, saat itu Madoka bersama temannya Sayaka sedang jalan-jalan. Mereka bertemu dengan Akemi, ia nampak mengejar seekor familiar (seperti seekor kucing) yang bernama Kyuubey. Kyuubey meminta tolong kepada Madoka dan menawarkan kekuatan kepada Madoka & Sayaka untuk menjadi seorang Puella Magi (Penyihir).
Dari sinilah Madoka mengetahui dunia yang berisi para Mahou Shoujo (Penyihir). Namun, mereka belum mengetahui konsekuensi dan kenyataan yang mengerikan menjadi seorang penyihir.
